Fitness Center Forms

Completing a fitness center release and health history form is required to have access to the fitness centers. Stop by the center to fill out the forms or print from the attachments below and turn into a fitness staff member.

 

Completed forms contain confidential health information and must be turned in to a fitness staff member. Please do not shuttle or leave forms at the fitness center.

If you are interested in participating in a Quad/Graphics recreation event, you will need to complete a recreational release form. This form can be shuttled, dropped off in the fitness center, or turned in to a fitness staff member.

Fitness Center Services

By appointment only 

Orientation

The fitness staff will introduce you to (or refresh your knowledge of) all of the equipment in the center to ensure correct and safe usage. Weight, set and repetition recommendations may be made. This is a requirement to use the center. 

 

Program Design

The fitness staff can provide you with a personalized program based on your goals, restrictions and preferences. We require initial appointments so we can introduce you to the program, set up baseline weights and ensure correct form. 

 

Fitness Assessment

The fitness staff will evaluate your current fitness level by measuring your BMI (body mass index), blood pressure, heart rate, flexibility, circumference and body fat. Once all of the tests are complete, we’ll explain the results to you. This appointment is strongly suggested for those who will be participating in program design. 
 

Personal Training

Personal training is for those individuals who are looking for more personal attention and motivation and who are ready to take their workouts to the next level. The fitness specialist will meet with you on a regular basis and take you through a personalized workout each time. Personal training is offered for a fee:

You may pay by cash, check, payroll deduction through the Quad/Card (up to $150 on one payroll) or credit/debit card.

Group Fitness

All Group Fitness classes are held in the fitness center or group fitness studio. Examples of classes we offer are Step, Kickboxing, Yoga, Pilates, Strength Training and Zumba. The usual cost for a session is $20. We hold four 11-week sessions throughout the year.
