Well You - Diabetes
The certified diabetic educator serves as the coach for patients enrolled in the Well You for Diabetes disease management program. The Well You for Diabetes program helps diabetics comply with best-practice clinical guidelines. These guidelines include normalization of metabolic parameters such as blood sugar, weight, LDL cholesterol and blood pressure, as well as maintaining medication compliance.

Well You - Asthma
The certified nurse educator serves as the coach for patients enrolled in the Well You for Asthma program. The program helps patients with asthma to live normal lives through knowledge about asthma and understanding how to best manage the condition. This includes strategies such as proper inhaler technique, understanding your triggers, and self monitoring skills. We work closely with your primary care provider to develop and comply with a personalized asthma action plan.

Well You - High Blood Pressure
The certified nurse educator serves as the coach for patients enrolled in the Well You for High Blood Pressure program. This program helps patients with high blood pressure learn how to make therapeutic lifestyle changes to help control their condition.
These include reduction of salt intake, balanced diet, stress management, smoking cessation, exercise and weight control, as well as maintaining medication compliance.
